    Overview

    Glaucoma is optic nerve damage usually associated with elevated intraocular pressure caused by impaired outflow of aqueous humor. Vision lost to glaucoma cannot be recovered, so all treatment is aimed at preventing further loss.

    Open-angle glaucoma is by far the most common. It is painless and gradual, quietly eroding peripheral vision until the patient notices tunnel vision late in the disease — which is why screening and adherence matter more than symptoms.

    Angle-closure glaucoma is the opposite: sudden severe eye pain, blurred vision, halos around lights, a fixed mid-dilated pupil, a red eye, headache and vomiting. It is an ophthalmologic emergency requiring pressure reduction within hours to save vision, and anticholinergic medications that dilate the pupil can precipitate it.

    Nursing interventions and rationales

    Pressure control

    • Administer prescribed drops on schedule: prostaglandin analogs, beta blockers, alpha agonists or carbonic anhydrase inhibitors.
    • Wait five minutes between different drops so the first is not washed out.
    • Monitor systemic effects, especially bradycardia and wheezing with beta blockers.
    • Support laser trabeculoplasty or surgical procedures with pre- and postoperative teaching and activity restrictions.

    Acute angle-closure response

    • Recognize sudden severe eye pain with halos, nausea and a fixed mid-dilated pupil as an emergency and notify ophthalmology immediately.
    • Give ordered agents to lower pressure rapidly: topical drops, oral or IV acetazolamide, osmotic agents such as mannitol.
    • Keep the patient supine with the head slightly elevated in a dim, quiet room and give antiemetics for vomiting.
