    Overview

    Dengue is a mosquito-borne viral illness transmitted by Aedes aegypti, which bites during daylight hours. Most infections cause a self-limiting febrile illness, but a subset progresses to dengue hemorrhagic fever and dengue shock syndrome, where plasma leaks out of the capillaries and platelets fall, producing bleeding and circulatory collapse.

    The illness has three phases the nurse must be able to name. The febrile phase brings high fever, severe retro-orbital headache, intense muscle and joint pain — the old name 'breakbone fever' — and rash. The critical phase begins as the fever falls, around day 3–7, and lasts 24–48 hours: this is exactly when plasma leakage peaks and the patient can deteriorate while appearing to improve. The recovery phase brings fluid reabsorption, when overhydration causes pulmonary edema.

    Treatment is supportive and hinges on careful fluid balance. Nursing care is serial hematocrit and platelet monitoring, meticulous hemodynamic assessment, bleeding precautions, and above all recognizing the warning signs of the critical phase before shock arrives.

    Nursing interventions and rationales

    1. Monitoring for the critical phase

    • Chart the fever curve and increase the intensity of monitoring as the temperature falls — this is when deterioration happens.
    • Assess vital signs, especially pulse pressure, every 1–4 hours depending on severity; a narrowing pulse pressure precedes hypotension.
    • Report the warning signs promptly: severe abdominal pain, persistent vomiting, mucosal bleeding, lethargy or restlessness, and a tender enlarged liver.
    • Follow serial hematocrit and platelet counts and interpret them together; a rising hematocrit with a falling platelet count means leakage.
    • Assess for pleural effusion and ascites: dyspnea, dullness to percussion, increasing abdominal girth.

    2. Managing fluid balance

    • Give isotonic crystalloid at the prescribed rate, titrated to hematocrit, urine output and perfusion rather than a fixed volume.
    • Maintain strict hourly intake and output and weigh daily on the same scale.
    • Encourage oral fluids in mild cases — oral rehydration is preferred when the patient can tolerate it.
    • Watch closely for overload during the recovery phase: crackles, dyspnea, puffy eyelids, rapid weight gain — fluid rates must be reduced as reabsorption begins.
    • Report a hematocrit that falls sharply with hemodynamic instability, which may indicate bleeding rather than improvement.

    3. Bleeding precautions

    • Assess for gum bleeding, epistaxis, petechiae, hematuria, hematemesis and melena each shift.
    • Minimize venipunctures, use small needles and hold pressure for at least 5 minutes.
    • Avoid intramuscular injections, rectal temperatures and nasogastric tubes unless essential.
    • Provide a soft toothbrush and avoid vigorous nose blowing.
    • Give platelet or blood transfusion only as ordered for significant bleeding, not for a low count alone.

    4. Fever and comfort

    • Give acetaminophen for fever and pain within maximum daily limits; document that no NSAIDs or aspirin have been given.
    • Use tepid sponging and light clothing rather than cold baths, which cause shivering.
    • Provide a quiet, dim environment for severe headache and photophobia.
    • Encourage rest and assist with all activity while the patient is weak and at fall risk.

    5. Prevention and teaching

    • Use bed nets or screens during the febrile phase, since the patient is viremic and can infect biting mosquitoes.
    • Teach elimination of standing water: buckets, tires, plant saucers, gutters and water storage containers.
    • Advise repellent use, long sleeves and daytime protection, since Aedes bites during the day.
    • Teach that a second dengue infection with a different serotype carries higher severe-disease risk, so prevention remains important after recovery.

    Patient and family teaching

    • Return immediately for severe abdominal pain, repeated vomiting, bleeding gums or nose, black stools, restlessness, unusual sleepiness or cold clammy skin.
    • Watch most carefully in the 24–48 hours after the fever breaks — this is the dangerous period.
    • Use only acetaminophen; never take aspirin or ibuprofen.
    • Drink oral rehydration fluids steadily and record how much you are drinking and passing urine.
