Serotonin syndrome — due to interaction between citalopram and St. John's Wort. Citalopram (SSRI) increases synaptic serotonin by blocking reuptake. St. John's Wort also increases serotonin levels. Combining these two agents dramatically elevates serotonin activity, risking serotonin syndrome — a potentially life-threatening condition marked by agitation, tremors, diaphoresis, tachycardia, hyperthermia, and clonus.
