Lymphatic structures serve as the primary site for antigen presentation and immune filtration. In the absence of an immunological challenge, the nodes remain small and soft. The connective tissue and overlying adipose usually obscure these structures during superficial palpation. Presence of palpable nodes suggests recent or active lymphatic drainage of pathogens. A. Shotty: This term describes small, mobile, firm nodes that feel like "buckshot" under the skin. While common in children following minor infections, they are not the standard expected finding for a healthy adult. They represent chronic, benign reactive changes rather than the normal non-palpable state. B. Large, firm, and fixed to the tissue: These characteristics are highly pathological and suggestive of malignancy or advanced granulomatous disease. Normal nodes should never be fixed to underlying structures. Such findings require immediate diagnostic investigation and possibly a biopsy to rule out neoplastic infiltration. C. Nonpalpable: In healthy adults, most lymph nodes cannot be felt during a physical examination. The absence of palpable nodes indicates that the lymphatic system is not currently responding to significant infection or inflammation. This is the expected baseline finding for a standard head and neck assessment. D. Rubbery, discrete, and mobile: While mobile and discrete are positive signs, a "rubbery" consistency is often associated with lymphoma. While some healthy people have small, palpable mobile nodes in the inguinal or cervical regions, "nonpalpable" remains the most technically correct "normal" finding for a healthy adult.
