    Anemia is a reduction in circulating red blood cells, hemoglobin or hematocrit below normal for age and sex, which lowers the oxygen-carrying capacity of the blood. It is a sign, not a diagnosis: the nursing question is always why. The three broad mechanisms are decreased production (iron, vitamin B12 or folate deficiency, chronic kidney disease with low erythropoietin, bone marrow failure), increased destruction (sickle cell disease, thalassemia, autoimmune hemolysis, mechanical valves) and blood loss (trauma, surgery, gastrointestinal bleeding, heavy menstruation).

    Morphology narrows the cause quickly. Microcytic hypochromic cells point to iron deficiency or thalassemia; macrocytic cells point to vitamin B12 or folate deficiency; normocytic cells point to acute blood loss, chronic disease or hemolysis. Iron deficiency is the most common anemia worldwide and in adults is frequently the first clue to occult gastrointestinal bleeding, so unexplained iron deficiency in an adult always warrants a source hunt.

    Symptoms come from tissue hypoxia and compensation: fatigue, pallor, dyspnea on exertion, tachycardia, dizziness and headache. Specific patterns add clues — spoon-shaped nails and pica in iron deficiency; a beefy red sore tongue with numbness, tingling and unsteady gait in vitamin B12 deficiency (neurologic damage that folate will not reverse); jaundice and splenomegaly in hemolysis. Severe or rapid-onset anemia can precipitate angina and heart failure in older adults.
