    Overview

    Deep vein thrombosis is a clot forming in a deep vein, most often in the calf or thigh. It develops from Virchow's triad — venous stasis, endothelial injury and hypercoagulability — and the most dangerous consequence is embolization to the pulmonary circulation, which can be fatal within minutes.

    Classic findings are unilateral calf or thigh pain, swelling, warmth and erythema, but a large proportion of clots are silent, and the first manifestation may be a pulmonary embolus. That is why prophylaxis in immobile, postoperative and hospitalized patients is a core nursing responsibility rather than an optional extra.

    Once a clot is confirmed, treatment is anticoagulation to stop propagation while the body's own fibrinolysis resolves the clot. The nurse's job becomes preventing embolism, delivering anticoagulation safely, watching for bleeding, and preparing the patient for months of therapy and lifelong prevention of post-thrombotic syndrome.

    Nursing interventions and rationales

    1. Preventing embolization and monitoring for PE

    • Do not massage, rub or perform vigorous range of motion on the affected limb; handle it gently during care.
    • Assess respiratory status frequently during the first days — sudden dyspnea, pleuritic pain, tachycardia, apprehension or hemoptysis suggest embolism.
    • If PE is suspected, stay with the patient, raise the head of the bed, give oxygen, notify the provider immediately and prepare for rapid anticoagulation.

    2. Relieving pain and swelling

    • Give analgesics as ordered, avoiding NSAIDs and aspirin in anticoagulated patients unless specifically prescribed.

    3. Administering anticoagulation safely

    • Give unfractionated heparin by continuous infusion on a pump with an independent double check, monitoring aPTT and adjusting per protocol; keep protamine sulfate available.
    • Administer low-molecular-weight heparin subcutaneously into the abdominal fat pad, at least two inches from the umbilicus, without aspirating or massaging, rotating sites.
    • Monitor platelet count for heparin-induced thrombocytopenia — a fall of more than half, usually 5–10 days in, requires stopping all heparin, not just reducing it.
    • Overlap warfarin with heparin until the INR is therapeutic for two consecutive days, since warfarin takes several days to take effect.

    4. Monitoring for bleeding

    • Assess for gum bleeding, epistaxis, bruising, petechiae, hematuria, tarry stools, prolonged bleeding from puncture sites and unexplained back or abdominal pain.
    • Monitor hemoglobin, hematocrit, platelet count and coagulation studies at the ordered intervals.
    • Report a sudden headache, confusion, visual change or one-sided weakness at once — these may signal intracranial hemorrhage.
    • Implement bleeding precautions: electric razor, soft toothbrush, no flossing if gums bleed, no rectal temperatures or suppositories, fall prevention.
