    Hypothyroidism is deficient thyroid hormone. In adults the most common cause worldwide is iodine deficiency and, in iodine-sufficient regions, Hashimoto's autoimmune thyroiditis; treatment-induced hypothyroidism after radioactive iodine or thyroidectomy is also frequent. Because thyroid hormone sets basal metabolic rate, the presentation is a body running too slow: fatigue, cold intolerance, weight gain, constipation, bradycardia, dry skin and slowed thinking.

    Symptoms develop insidiously and are commonly attributed to aging or depression, which delays diagnosis. In older adults, mental slowing, falls and unexplained heart failure may be the presenting features rather than the classic picture.

    Untreated severe hypothyroidism can progress to myxedema coma — hypothermia, profound bradycardia, hypoventilation with CO2 retention, hyponatremia, hypoglycemia and decreasing consciousness, typically precipitated by infection, cold exposure, sedatives or stopping replacement therapy. It carries high mortality and is a medical emergency.

    Nursing interventions and rationales

    1. Hormone replacement and cardiac safety

    • Give levothyroxine once daily on an empty stomach with water, at the same time each morning, and hold breakfast for 30–60 minutes.
    • Expect gradual improvement over weeks; explain that a missed early response does not mean the dose is wrong.
    • Monitor apical pulse before each dose during titration and report tachycardia, palpitations, chest pain or new nervousness as signs of over-replacement.
    • Anticipate lower starting doses and slower increases in older adults and those with coronary disease, because a rapid rise in metabolic rate can precipitate angina or infarction.

    • For suspected myxedema coma, maintain the airway, support ventilation, give IV thyroid hormone, fluids, glucose and corticosteroids as ordered, and correct temperature slowly.

    3. Managing constipation and nutrition

    • Assess bowel pattern daily; slowed peristalsis can progress to impaction or ileus.
    • Encourage fluids and gradually increased fiber, along with activity as tolerated.
    • Give stool softeners rather than routine stimulant laxatives, and check for impaction if there is no stool for three days.
    • Provide a low-calorie, nutrient-dense diet during weight gain, explaining that weight will normalize as the dose becomes therapeutic rather than through severe restriction.
    • Monitor cholesterol, since hypothyroidism raises lipids and cardiovascular risk.

    4. Energy, cognition and mood

    • Plan care around rest periods and space activity to match tolerance.
    • Give clear, simple, repeated instructions and provide written material; processing is genuinely slowed and improves with treatment.
